The Agency Self-Assessment tool is intended to help agencies that are making the transition to a Qualified Residential Treatment Program (QRTP) and those that have become a QRTP. Developing and maintaining a QRTP Trauma-Informed Model requires the transformation of processes, people and systems throughout the organization.

This form is intended for service provider agencies – they must submit a description of their Trauma-Informed Care Model to the Colorado Department of Human Services (CDHS) Office of Behavioral Health (OBH) upon application for a Qualified Residential Treatment Program (QRTP) license and when applying for a mental health designation through OBH.

Full checklist of the rules at 7.714, Quality Standards for 24-Hour Child Care, which all facilities this rule set applies to must be in compliance with prior to a new license being issued. 7.714 applies to specialized group facilities, residential child care facilities, shelter residential child care facilities, and psychiatric residential treatment facilities. Section 7.714.53, et seq., and the applicable definitions in Section 7.714.1, also apply to approved family foster care homes and day treatment centers.
