Colorado’s five-year prevention plan reflects a vision of a child welfare system that improves the safety, permanency and wellbeing of all children, youth and families through a continuum of community-based prevention services and supports.

Colorado facilities will complete this form and submit it to their assigned Licensing Specialist at the Division of Child Welfare Provider Services Unit (PSU) with application for License if they plan to enroll with Health First Colorado.

This web link provides information on the Children and Youth Mental Health Treatment Act (CYMHTA) (C.R.S. 27-67-101, et. seq.), also called House Bill 18-1094, which allows for families to access mental health treatment services for their child or youth. CYMHTA is an alternative to child welfare involvement when a dependency and neglect action isn’t warranted. CYMHTA funding can be available when there is no other appropriate funding source for treatment, such as private insurance.

The Rocky Mountain Human Services (RMHS) Momentum Program supports the transition of children and adults from inpatient mental health institutes, hospitals, home and other care settings to community living.
