Form to be completed and submitted to the Provider Services Unit for a change in Sponsoring Agency for a Specialized Group Facility when the separate governing body remains unchanged

Application form for use when a licensee has previously held a permanent license which was changed to a probationary license and the licensee wants to apply for the permanent license to be restored. This form is also used when the licensee would be issued a permanent license based on their service type but is temporarily unable to comply with all requirements and a provisional license is issued. The licensee would use this form to apply for the permanent license.
